When the IRS notifies a client that his or her installment agreement will be terminated, practitioners need to be ready for the client with answers and potential courses of action.

In this template, practitioners fill in their unique firm information and taxpayer details to request that the installment agreement be reinstated. The letter can also be used to request a Collection Appeals Program (CAP) hearing in the event the agreement is not reinstated and/or the IRS is enforcing other collection
