Under the automatic disciplinary provisions of the Institute’s bylaws, Mr. Woldorf was admonished, effective August 9, 2012, as a result of the Consent Order that he entered into with the Arizona State Board of Accountancy. Specifically, the state board placed Mr. Woldorf’s CPA certificate on probation, for a period not to exceed twelve months, in connection with his negligence in the preparation of a client’s 2005 and 2006 personal income tax returns.
